A Clinical Study of Whipple¢¥s Operation





Abstract
This report of 52 cases undergoing pancreaticoduoenectomy at the Department of General Surgery of Busan National University Hospital during 11 and 1/2 years from Jan. 1963 to Jun. 197.4 were, reviewed to determine what effect softie various factors and conditions of patients in performing- this procedure had, on the rate of operative mortality and morbidity.
And the following results were obtained:
Patients with obesity, heart disease, alcoholism, increased bilirubin value and prothrombin time, roentgenological evidence of tumor invasion of the duodenum, and an operative finding of a nondilated common duct had significantly higher incidence of operative mortality and morbidity in this procedure.
The age itself on the patient made no difference to mortality and morbidity.
Postoperative gastrointestinal bleeding was less uncounted if a vagotomy or greater than 50% gastrectomy was performed.
The operative mortality rate was 17.3% overall.
The 5-year survival rate was 21.1% overall (16.7% for carcinoma of pancreas and 28.6% for carcinoma of billiary system).
As a result of this review the following recommendations are made; the pancreaticoduodenectomy should be performed for selected patients or under the conditions corrected as possible and not categorically denied all patients.
